included on the timeline.  If you have facts which would contradict any
information on the timeline, you may also use this form to submit your feedback.
Any feedback is encouraged.  
Documentation could include references from a local or state association
minute book and the ABA minute book which carries the name of a church or
churches that fellowship in the local or state association.  The associational
meeting place and page numbers would be helpful.  If you were personally
involved in the organization meeting of a local or state association your
personal knowledge of the events may be adequate documentation.  
Recognized church history books may also be used as documentation.
Scanned or photocopies of the documentation may be requested.
